I herby give up all my rights to sue or make any claim for damages due to negligence or any other reason whatsoever against the American Motorcyclist Association and its district organizations, the promoters, sponsors, landowners and all other persons, participants or organizations conducting or connected with Sportsman this event for injury to property or person I may suffer, including crippling injury or death, while participating in the event and while upon event premises. I know the risks of danger to myself and my property while preparing for and participating in the event and while upon the even premises and relying upon my own judgment and ability, assume all such risks of loss and hereby agree to reimburse all costs those persons or organizations connected with this event for damages incurred as a result of my negligence.

No rider under the legal age of majority in the state in which the meet takes place may compete without the written consent (signature on liability releases and entry forms) of his/her parents or legal guardians present at the meet. An authorized adult acting on behalf of the parents or legal guardians must provide a notarized statement indicating they have been given the authority by the parent or legal guardian to be responsible for the minor guring the meet. All releases and notarized statements must be forwarded to the AMA with the referee report and/or injury report. The parents, legal guardians or authorized adult must remain present while the AMA member he/she is responsible for is at the meet.

1. Required information and signatures must be provided at meet.
2. AMA membership cards must be prosented at each event or contestant must join the AMA at the event.
3. All checks must be negotiable. Contestants submitting non-negotiable (i.e. insufficient funds or stop-payment) checks will be considereed non-starters for that event and will not be accepted for subsequent events until the matter is resolved. Any bank charges incurred will be paid by the contestant.

1. Entry fee is $75.00 (U.S.) for each round. Minder fee is $40.00 for each round. ITS Youth pre-entry for one day is $20.00.
2. LATE ENTRIES: May be made after the deadline dates until closed by the organizer. Late entry fee is $100.00 and will be enforced, $50.00 for Minders, $20.00 for ITS Youth one day.
3. For mail-in entries: complete one entry form for each round entered (send the top half only) and please send only one check totaling correct amount for all requested entries. DO NOT STAPLE CHECK TO FORM!)

Requests must be in writing or by e-mail (info@natcentry.org) to NATC Entry. Requests postmarked up to 2 weeks after event will be refunded in full for minders and riders.

Event information and confirmation of acceptance will be returned for each entry.

All riders must wear a suitable garment with rider's last name on the back. Requirements for location, size and color of lettering will be sent with entry confirmation. NOTE: All machines must display a proper number plate.

Not for National Championship Points. "Little league parents" NOT ALLOWED.
